Today we’re launching Adaptive Notes. When you edit a note in Tandem, the notes that follow carry those edits forward, so your notes come out closer to how you write.
Every clinician writes differently
Two clinicians can see the same patient and write the note differently. One orders the sections one way, another in a different order. One writes the plan as bullet points, another in short paragraphs. Some want a lot of detail in the history, others very little.
Templates give a note its structure, and Memories let you state a rule such as “always list allergies first”. But much of how you write is hard to put into a rule. That is where Adaptive Notes comes in: it picks up the patterns in how you edit, so you don’t have to write them down.
Edit your notes as usual, and the next ones adapt
There is nothing to set up. Tandem learns from your edits and applies them to later notes. It adapts how a note is written, not the facts in it:
Structure: how you order the sections
Wording: the phrasing you prefer
Detail: how much you want in each part
Adaptation is per clinician and per template. Your edits only shape your own notes with that template, and each template adapts on its own. A colleague’s notes are never affected.
How it fits with Templates and Memories
Templates set the structure for each kind of visit.
Memories hold rules you want applied every time, such as “always list allergies first”. Clinic admins can set Clinic Memories for the whole team.
Adaptive Notes follows the patterns that are hard to put into a rule, such as phrasing, order and level of detail. Use Memories for anything that must always or never happen, and let Adaptive Notes handle the rest. Read more about Memories and templates
You stay in control
Every note is still a draft you review, and nothing reaches the medical record until you approve it. A note that sounds like you is still AI-generated and can contain errors, so always review it before approving. You can turn Adaptive Notes off at any time in Settings → General.
Tandem’s AI Scribe is CE marked as an EU MDR Class IIa medical device. Read how we handle data and security.
